Thank you for your recent purchase from Teamlogo.com. Please be advised that we ship our products using UPS and the USPS. These are both very fine companies however they "RARELY" handle packages with kid gloves and from time to time the contents of packages become damaged in transit. This form is provided to help us to help you. It is important that if you have received a damaged item that you fill in this form completely so that we may make the necessary arrangements to report the damage to the appropriate carrier.
What to expect: Once you have filled in this form we will file a damage report with the appropriate shipper. Please be patient as we file the claim and wait for a response from the shipper. The shipper has the right to come to your location and retrieve the package and contents and/or inspect the items. The shipper may also waive their right to inspect the package and/or contents. We will keep you advised of the shipper's decision. The shipper will then notify us of their findings and then we can work to get your replacement product on it's way to you. In the event that the damage was determined to be the result of an action taken by the customer no replacement will be shipped and no refunds will be given.
